Project Description

The CBTPP is envisioned to become a community-based outreach program of the USC UCEDD at CHLA. 

The Transition Pilot Plan Components:

Based on a community-integration approach provides service coordination for full range of transition services and supports that include:
  • Health care
  • Social Supports 
  • Leisure 
  • Career Guidance 
  • Housing
  • Education
  • Financial Literacy
  • Employment

Transition Pilot Plan Characteristics


  • Sustainable
  • Is equitable and comprehensive
  • Feasible for implementation
  • Meets the needs of ID/DD target population
  • Can be easily implemented
  • Blueprint including pilot partners available for CHLA UCEDD implementation

The goals/outcomes of this pilot program is to:

  • to develop an equitable, comprehensive plan for a community-based transition pilot program (CBTPP) for youth and young adults (YYA) with ID/DD in an underserved and racially and ethnically diverse Los Angeles community that addresses both transfer of medical care as well as transition into community-based services and supports to promote independent living and employment

  • Increase awareness of the transition needs of YYA with ID/DD

  • Interagency collaboration through Partnership Advisory Team (PAT) 

  • Dissemination of transition resources to CBTPP and USC UCEDD network
